NRC-06-96-300 Modification No. 5 Page 2 of 3 Modification to: 1) revise completion dates stated under Article F.2 PRODUCTION AND DELIVERY SCHEDULE - MAJOR TASKS, item number 4; 2) Delete the term of the contract stated in Article F.3 and substitute an extended term thereof.

Accordingly, the contract is hereby modified:

1. Under section F. Article F.2 PRODUCTION AND DELIVERY SCHEDULE - MAJOR TASKS, item numbers 3(i), 3(j), and 4 are hereby modified as follows:

3. Production (i) The contractor is to deliver 02/27/97 the entire complete revision of the rough cut video.

(j) The NRC Project Officer will be 03/04/97 allowed three (3) working days for inspection and acceptance / l rejection of the entire complete i revision of the rough cut video. l l

4. Final Video Delivery  !

l (a) The contractor is to deliver 03/14/97 i the final narrated video. l (b) The NRC Project Officer will be 03/28/97 I allowed ten (10) working days for  ;

inspection and acceptance / rejection of the final narrated video.

2. Section F. Article F.3 DURATION OF CONTRACT PERIOD (MAR 1987) is deleted in its entirety and the following is substituted therefor:

This contract shall comence on January 2,1996 and will expire on April 4, 1997.

3. This action does not increase or decrease the total amount of the contract.

4. All terms and conditions of the contract remain the same.
