De enclosum to this letter is a mvised Nuclear Regulatory Conunission (NRC) Form 396, "Cenification of Medical Examination by Facility Licensee," for Mr. Michael C. Bottomley

(License Number OP-31058, Docket Number 55-32236). His revised form certifies that Mr. Bottomley's eyesight has changed such that he is now required to wear corrective lenses while performing licensed activities. De revised fonn is being submitted pursuant to 10 CFR 55.25 and 10 CFR 50.74(c), within thirty days ofleaming of the diagnosis.
Consequently, his license should be conditioned to reflect this restriction.
