UNIT 1 NOS.11 AND 12 MAIN STEAM HEADER RADIATION MONITORS - SPECIAL REPORT We submit this Special Report concerning the inoperable Unit 1 Main Steam Header Radiation Monitors as required by Technical Specification 3.3.3.1, Table 3.3-6, Item 2.b.ii.
ACTION TAKEN Numbers 11 and 12 Main Steam Header Radiation Monitors were removed from OPERABLE status on February 8,1997 at approximately 0805 to allow installation of new Main Steam Header Radiation j
Monitors per an approved modification. Actions not requiring the monitors to be removed from service were completed prior to removing the monitors from OPERABLE status. However, unanticipated -
problems have delayed completion of the modification, post-modification testing, and declaring the new monitors OPERABLE. Additional design evaluations have been completed and design instructions i
issued to allow the installation of the modification in an alternate manner.
EFFECT ON OPERATION Unit 1 Technical Specification 3.3.3.1 Action Statement (30) was entered when Nos. I1 and 12 Main Steam Header Radiation Monitors were removed from service. in accordance with this Action Statement j
l and our Accidental Radioactivity Release Monitoring and Sampling Methods procedure (ERPIP-821),
alternate sampling methods were implemented. The inoperability of Nos. I1 and 12 Main Steam Header Radiation Monitors did not affect Unit I operation.
CAUSES OF INOPERABILITY The cause of the inoperability was the removal of the monitors from service to complete an approved modification.
PLANS AND SCHEDULES FOR RESTORING THE SYSTEM TO OPERABLE STATUS The Nos. I1 and 12 Main Steam Header Radiation Monitors were removed from OPERABLE status on February 8,1997 at approximately 0805. Additional design evaluations have been completed ud design instructions issued to allow the installation of the modification in an alternate manner. Physical work is on-going which will be followed by post-modification testing and declaring the new monitors OPERABLE. The estimated completion date for the return of the monitors to OPERABLE status is March 7,1997.
