Dear Mr. Denton:

In response to Mr. T. M. Novak's letter of July 24, 1985 requesting that we

"... provide information as to the demand and failure frequencies of the isolation function such as acquired from test and maintenaner activities (i.e., provide the demand and failure frequencies relating to challenges to the electrical and mechanical components of the UHI System upon actuation of isolation signals and identify the associated cause for each failure)."

We have reviewed our records and have retrieved the following data.

Catawba, Unit 1 7 system demands 0 failures McGuire, Units 1 and 2 25 system demands 1 failure i

The failure at McGuire occurred on Unit 1 on October 31, 1984 as the result of incorrectly installed accumulator level transmitters (Unit 2 transmitters were correctly installed). Also at this time, it was discovered that the j

transmitters were miscalibrated and would have closed early. The Unit 1 l

transmitters were teinstalled correctly and the transmitters on both units i

were recalibrated. For further detail, please refer to Licensee Event Report 369/84-30 dated December 3, 1984 and supplemented by letter of December 19, l

1984; also reference Civil Penalty: EA84-130.
