NRC INTEGRATED INSPECTION REPORT 50-271/96-09

Dear Mr. Reid;

i

On October 19,1996, the NRC completed an inspection at your Vermont Yankee reactor

facility. The enclosed report presents the results of that inspection. During the 7-week

,

I

period covered by this inspection period, your staff's conduct of activit ec at the Vermont

,

Yankee f acility was generally characterized by safety-conscious operations., sound

l

l

engineering and maintenance practices, and careful radiological work contrels during a

l

!

particularly challenging refueling outage.

l

i

An integration of the inspectors' observations and findings from the various functional areas

resulted in an overall assessment that procedural adequacy and adherence were a recurring

,

performance concern. We acknowledge your staff's recognition of this area of concern,

t

identified via the plant self assessment processes, and we encourage your prompt attention

to reverse this negative trend.

!

In the area of plant support, we found an overall effective program for occupational

i

radiation protection during the refueling / maintenance outage. The emergency preparedness

'

program was reviewed and found to be appropriately implemented and directed towards

l

public health and safety.

An inspection of the inservice inspection activities conducteo during the refuel outage

identified good engineering and non-destructive examination practices with good oversight

!

l

and interaction with the subcontractor. Review of the preparations and modification

I

documentation for the core shroud repair found the repair plans in conformance with

l

regulatory guidelines.

!

'

Luring the electrical maintenance inspection conducted the week of September 16,1996,

the inspector found an inadequacy in the station battery service discharge surveillance test.

l

The failure to establish an acceptance criteria related to the design basis of the DC system

was a violation of Technical Specification 6.5.A.6. This violation indicated poor

communications between the design engineering and maintenance engineering staffs and a

failure of station procedures to adequately assure the capacity of the main station batteries.

surrounding the violation are described in detail in the enclosed report. Please note that you

are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part, to

determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and

its enclosures will be placed in the NRC Public Document Room.
