RANDOM i
l On August 21,1996, a licensee employee experienced a confirmed intoxilyzer positive test result. The employee was placed on indefinite suspension, denied unescorted access

i and referred to the Employe'e Assistance Program (EAP) for evaluation and treatment
}
recommendations.
The employee was returned to work on September 24, 1996,
- completed the recommended rehalailitation program, and unescorted access was reinstated on October 1,1996. The employee is presently participating in a Follow-up p ogram.
l PRE-ACCESS l
On October 7,1996, a licensee employee tested positive for methamphetamines under
" pre-access" on his return-to-work from a long-term illness absence. Upon receipt of the confirmed positive test result, the Medical Review Officer requested, in writing, that d &1 testing be performed on the aliquot sample.
Results proved 96% D-Methamphetamine and, with this result, was adjudicated as a confirmed positive test result. The employee was placed on indefinite suspension, denied unescorted access and referred to the EAP for evaluation and treatment recommendations.
i On November 25,1996, the employee was returned to work, by recommendation of the EAP, while continuing the intensive out-patient program. On " pre-access" testing, the employee experienced a confirmed positive intoxilyzer test result.
The employee i
requested a blood alcohol confirmation which noted a positive result. The employee was placed on indefinite suspension and, after carefully reviewing and considering all facts, it was the Company's decision to terminate employment effective November 25,1996.
Unescorted access was never reinstated.

FOLLOW-UP OF PREVIOUS SIX-MONTH REPORT FOR-CAUSE OBSERVED BEHAVIOR On July 13, 1995, a licensee employee experienced a confirmed intoxilyzer positive in the For-Cause, Observed Behavior program.
This employee had experienced two disciplinary suspension occasions in the previous two years. The employee had returned to work in March,1995, under the condition that he would participate in an evaluation by the EAP and comply with any recommendations resulting frorn,the evaluation. The employee's return to work criteria included dmg/ alcohol testing at the discretion of Management, which was implemented on July 13, 1995, when he failed to report for work as scheduled. The employee was placed on indefinite suspension and unescorted access denied on July 13, 1995, for a confirmed positive test result. On July 25,1995, employment was terminated, due to his overall employment record.
An arbitration hearing was held on February 6,1996, with a decision on April 18,1996, that the employee be evaluated under the normal procedures for evaluating fitness for return to work and determining the rehabilitation program that he must enter and successfully complete.
UPDATE On May 24,1996, the employee was evaluated by the EAP and referred to Gateway Rehabilitation Center. The employee initiated an intensive outpatient evening program o
on June 3,1996, which was successfully completed on July 18,1996. Unescorted access was reinstated and the employee was placed in the Follow-up program.
