SUMMARY

OF OPERATIONS JUNE, 1985 i

During the month of June, there were two (2) automatic' shutdowns which occurred on 6-8-85 and 6-23-85.

The following major safety-related maintenance was performed in'the

. month of June:

1.

Miscellaneous corrective and preventive maintenance was 4

performed on the diesel' generators.

d 2.

Replacement of containment tendon field anchors is complete.

-3.

Two control rod drive system cables were repaired. An electrical short had occurred in the containment electrical penetration causing the reactor trip on 6-23-85.

3 85-010-00 AB BU A reactor' trip occurred due to under-frequency on the reactor coolant pump (RCP) buses. The underfrequency condition occurred because the RCP bus power sources had not been realigned to the startup transformers prior to trip-ping the main turbine.

This event was caused by personnel error and procedural inadequacy. The individuals involved i

have been counseled.

The appropriate procedures have been changed to prevent recurrence.
