004 850600 F 25.2 G 3 85-010-00 AB BU A reactor' trip occurred due to under- '

frequency on the reactor coolant pump (RCP) buses. The underfrequency condition occurred because the RCP bus power sources had not been realigned to the startup transformers prior to trip-ping the main turbine. This event was caused by personnel error and procedural inadequacy. The individuals involved i

have been counseled. The appropriate procedures have been changed to prevent recurrence.

005 850623 F 24.1 A 3 85-012-00 AA CBL4 A reactor trip occurred due to an elec-trical short between two control rod drive system ca,bles which were routed

through the same containment electrical penetration. The cables have been re-paired and re-routed.
