SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-254/265/96012
Dear Mr. Kraft:
On September 23, 1996, the NRC completed an inspection at your Quad Cities
Nuclear Power Station facilities. The enclosed report presents the results of
that inspection.

During the period covered by this inspection report, your staff
inappropriately invoked NRC generic letter guidance, in lieu of following
'
Technical Specification (TS) requirements, when it was believed that a
required surveillance test had been missed. Although it was ultimately
determined that the test was not required, the failure to implement a TS
action statement which was believed to be applicable is a significant concern.
We are also concerned that members of your Operations Department were not
familiar with the response characteristics of the Electro-Hydraulic Control
system at low pressures, were not familiar with the Reactor Core Isolation
Cooling system condenser drain valve operation, and lacked sensitivity to
potential seismic hazards associated with outage work.
In several instances
when problems were encountered, your staff did not utilize the Problem
Identification Form (PIF) system to capture the issue for documented
evaluation and corrective action.
Use of PIFs was not mandatory, but non-use
was considered a weakness because of the lost opportunity to ensure thorough
review and evaluation, learn applicable lessons, and prevent recurrence.
The inspectors identified violations of regulations concerning procedure
application and content.
Specific examples involved an annunciator procedure
which contained an impossible instruction, and failure to include quantitative
or qualitative acceptance criteria for critical dimensions of safety-related
equipment in maintenance and test procedures.
The operability of a Main Steam
Isolation Valve, a Residual Heat Removal Service Water Pump, and an off gas
monitor were affected.
The violation is of concern because of your continuing

problems with maintaining safety-related equipment available and reliable.

The inspectors identified examples of inadequate post-maintenance tests and
maintenance related equipment problems which also appear indicative of
'
weaknesses in your maintenance program.

The violations are cited in the enclosed Notice of Violation, and the
circumstances surrounding the violations are described in detail in the
enclosed report.
Please note that you are required to respond to this letter
and should follow the instructions specified in the enclosed Notice when
preparing your response.
The NRC will use your response, in part, to
determine whether further enforcement action is necessary to ensure compliance
with regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of
this letter and its enclosures will be placed in tha NRC Public Document Room
(PDR).
