APPENDIX A NOTICE OF VIOLATION Washington State University Docket No. 50-27 Triga Research Reactor License No. R-76 Nuclear Radiation Center As a result of the inspection conducted during the period of July 15-17, 1985, and in accordance with NRC Enforcement Policy 10 CFR, Part 2, Appendix C, the following violations were identified:

A. Technical Specifications, Section 6.8, " Operating Procedures" states in part, " Operating procedures shall be in effect for the following items:...(6) performing preventative maintenance and calibration tests on the reactor and associated equipment." Technical Specifications, Section 6.9, " Facility Operating Records" requires that records and logs shall be prepared and maintained for principal maintenance activities involving equipment and component surveillance activities required by the Technical Specifications. Further, TS, Section 4.3.4, states that the reactors ventilation system absolute filter shall be changed at least every two years.

Contrary to the above, as of July 17, 1985; no procedures have been established for accomplishing the absolute filter changeout, and no records were maintained of the reactor's ventilation system absolute filter change outs.

This is a Severity Level IV Violation (Supplement 1).

B. Technical Specifications, Section 6.5.4, " Reviews" states in part, "The responsibilities of the Radiation Safeguards Committee (RSC) or designated subcommittee thereof shall include, but is not limited to, the following...(8) biennial review of all standard procedures, the facility emergency plan, and the facility security plan."

Contrary to the above requirement, a biennial review of the facility's standard operating procedures have not been conducted since January 1983.

This is a Severity Level V Violation (Supplement 1).

C. 10 CFR Part 50.54 " Conditions of Licenses" states in part (1-1) that

" Holders of operating licenses in effect on September 17, 1973, shall

' implement an operator requalification program whic'h, as a minimum, meets

' the requirements of Appendix A of Part 55 of this chapter which was submitted for approval by the Atomic Energy Commission."

~'The licensee's NRC approved operator requalification program of August 20, 1974, requires that each licensed senior reactor operator (SRO) must actively participate in operation of the facility in order to 4

remain qualified. The approved program requires that records be maintained to document each individual's participation in the program.

Contrary to the above, during the period of January 1983 through the time of this inspection one SRO did not maintain records of his participation in the operators requalification program.

This in a Severity Level V Violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Washington State University is 4

hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken a'nd the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
