NOTICE-0F VIOLATION This is to inform the Commission that a Notice of Violation (Notice) will be issued on or about September 6. 1996, to Mr. Kirk Carroll, an unlicensed individuai employed by Energy Fuels Nuclear. Inc.. The action is based on an investigation which concluded that the individual deliberately violated the licensee's procedures by entering a yellowcake packaging enclosure without wearing a full face respirator.
The schedule of issuance and notification is:
Mailing of Notice September 6. 1996 Telephone Notification of Individual September 6, 1996 The individual has 30 days from the date of the. Notice in which to respond.
Unless the individual provides a response within 30 days justifying withdrawal of this action, in 45 days this action will be placed in the PDR and the State of Utah will be notified.
