NRC SPECIAL INSPECTION REPORT NO. 50-254/96015(DRS);
50-265/96015(DRS) AND NOTICE OF DEVIATION
Dear Mr. Krefc:
On November 6,1996, the NRC completed an inspection at your Quad Cities Nuclear
Power Station fecility. The enclosed report presents the results of that inspection.
Based on the results of this inspection, certain of your activities, set forth in the enclosure
'
to this letter, appear to be a deviation from commitments which you have made in previous
correspondence with the Commission regarding the trending of residual heat removal room
cooler cooling water differential pressure. A written response is requested.
Equipment trending weaknesses were also identified in a recent Engineering and Technical
Support inspection. Because equipment performance monitoring is an important method to
monitor potential degradation in the materiel condition of plant systems, weaknesses in
this ared identified in this report and in previous inspections warrant your attention.
In addition, we are concerned about the difference between conclusions reached regarding
the safety significance of undersized thermal overloads identified in the control room
ventilation system at your facility and the Dresden facility. As a result, we request that
you provide us, within 60 days of receipt of this letter, the results of a reassessment of the
safety significance of this issue and a justification for differences, if any, between the two
facilities.
Please note that you are requested to respond to this letter and should follow the
instructions specified in the enclosed Notice when preparing your response. The NRC will
use your response, in part, to determine whether further enforcement action is necessary
to ensure compliance with regulatory requirements.
