SUBJECT:
NRC INSPECTION REPORT NO. 50-341/96013 AND NOTICE OF VIOLATION
Dear Mr. Gipson:
On December 16,1996, the NRC completed an inspection at your Fermi 2 reactor facility.
The enclosed report presents the results of that inspection. During the inspection, the plant
operated safely.
,
Based on the results of this inspection, the NRC has determined that several violations of
NRC requirements occurred. These violations are cited in the enclosed Notice of Violation
(Notice) and the circumstences surrounding them are described in detail in the subject
inspection report. The violations are of concern because: 1) An internal baffle failure on
the Emergency Diesel Generator Number 12 muffler was improperly assigned a low

significance and corrective actions to repair the muffler were then delayed considerably
without reassessment. 2) A 10 CFR 50.E 9 Sefaty Evaluation of the Cycle 6 Core

Operating Limits Report failed to properly idert .y that a technical specification change was

necessary to ensure that mechanical fuel limits would be maintained during a rod
withdrawal error event. 3) An important safety function was not properly ratested due to
inadequate surveillance procedures following removal of an electricallead.
You are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
