Task Order No. 5. Modification No. 2 Under Contract No. NRC-04 50 Entitled " Individual Plant Examination of External Events (IPEEE)

Submittal Reviews" The purpose of this modification is to 1) incoroprate a within scope change

2) increase the contract ceiling by $191.172.89 from 134.260.83 to 325.433.72

3) increase the total amount obligated by $180.000.00 from $134.260.83 to

$314.260.83 and 3) extend the period of performance through December 31. 1997.

Task Order No. 5 is hereby modified as follows:

1.) Task Order No. 5 is modified to incorporate additional subtasks 3 and 4 which provides for the contractor to attend Senior Review Board. ACRS, and Commission Meeting in accordance with the attached statement of work (subtask 3).

The contractor will also continue its project management under subtask 4, which calls for the contractor to coordinate and make necessary arrangements for technical specialists to attend the associated NRC meetings.

2.) Paragraph two of the definitization letter dated May 4, 1995 is deleted in its entirety and replaced with the following:

"The Jeriod of performance for Task Order No. 5 is May 5, 1995 through Decem)er 31, 1997. The total cost ceiling is $325.433.72. The amount of

$312.917.04 represents the total reimbursable costs and the amount of

$12.516.68 represents fixed fee. The total amount obligated is $314.260.83.

Total FY94 Obligation Amount $134.260.83 Total FY97. Obligation Amount $180.000.00 Cumulative Total of NRC Obligations: $314.260.83 This modification obligates $180.000.00 in FY97 funds.

All other terms and conditions of Task Order No. 5 remain the same.

Please indicate your acceptance of this task order modification by having an-official, authorized to bind your organization, execute three copies of this document in the space provided and return two copies to the Contract Specialist. You should retain the third copy for your records.
