AND NOTICE OF VIOLATION

The NRC conducted an inspection of the Beaver Valley Power Station (BVPS) facilities from

August 13 to September 28,1996. The enclosed report presents the results of that

i

inspection.

!

!

During the six week period covered by this inspection period, your staff demonstrated

strong operational command and control during the Unit 2 shutdown and refueling

j

activities. Engineers effectively managed their workload and provided timely resolution to

t

various refueling outage issues. Senior management's review of emergency diesel

generator post maintenance testing weaknesses was insightful. Performance in the

j

radiation protection (RP) area by RP staff was good. Radiological cont'rols established

during the Unit 2 refueling outage were appropriate and well planned. Radiation worker

i

practices were improved compared to the previous Unit 1 refueling outage,

i

,

Engineering performance improved in quality of documentation, backlog reduction, and

response timeliness. Several recently identified issues were related to previous design

control prnblems. Examples within the past six months included AMSAC timing relay

reset, AMSAC feedwater flow instrument setpoints, fire pump capacity, and containment

j

penetration overpressure protection. Issue identification demonstrated a good questioning

l

attitude by your engineering staff. Notwithstanding, our inspectors identified a

longstanding valve misalignment in the Unit 1 reactor pressure control system. Your initial

I

conective actions were appropriate. The cause of this error and results of your expanded

scope review will be assessed during future inspections.

i

Based on the results of this inspection, two violations of NRC requirements were identified.

The first resulted from your failure to audit activities of the Onsite Safety Committee (OSC)

since 1992. Our inspectors noted the strengthened oversight provided by your recently

,

-

established Nuclear Safety Review Board; however, this does not in itself fulfill your

existing license requirements to perform and audit OSC activities. The second violation

was failure to adequately control safeguards documents.

you are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter
