Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction:
Nonconforming Terminations in CVI Corporation Equipment (SD-75)
In a February 13, 1985 telephone conversation between your Mr. T. Rebelowski and our Mr. R. K. McCarthy, Northeast Nuclear Energy Company (NNECO) reported a potential significant deficiency in the construction of Millstone Unit No. 3 as required by 10CFR50.55(e).
As noted in Reference (1), the potential significant deficiency involved improper terminations found in equipment supplied by CVI Corporation.
Termination deficiencies include crimps which were unacceptable per vendor's instructions, terminations crimped with the wrong tools, multiple conductors crimped in one lug, and incorrect lug size used.
In Reference (2) we determined that this condition represents a significant deficiency in the final design of Millstone Unit No. 3.
We also noted in Reference (2) that a 100% inspection of the lugs in the 2 remaining CVI panels (3 HVC* PNLFLT 1 A and IB) would be performed. Since this work is not yet completed, the corrective action and final report cannot be provided.
