NRC COMBINED INSPECTION REPORT 50-245/96-13; 50-336/96-13; 50-

423/96-13; and NOTICE OF VIOLATION
This letter refers to the routine safety inspection conducted by Dr. Jason C. Jang of this
office on November 12-22,1996, of activities authorized by NRC License Nos. DPR-21,
DPR-65, and NPF-49 at the Millstone Site, Waterford, Connecticut and to the discussion of

our findings held by Dr. Jang with Mr. S. Scace and other members of your staff at the
conclusion of the inspection.

Areas examined during this inspection, involving your radioactive affluent control
programs, are important to public health and safety and are discussed in the NRC'
l
inspection Report that is enclosed with this letter. Within these areas, the inspection
l
consisted of selective examinations of procedures and representative records, interviews
with personnel, and observations by the inspector.
Within the scope of this inspection, we noted that your staff maintained effective programs
for the control and monitoring of radioactive liquid and gaseous effluents. We further
noted that radiation monitoring system calibration techniques for Unit 2 had improved
significantly since our last review of this area. Surveillance test results for air cleaning
systems required by Technical Specifications (TS) at all three units were good. However,
two of your activities were determined not to be in compliance with NRC regulations, as
outlined in the attached Notice of Violation. The violations include: (1) failure to establish
and implement procedures as required by Technical Specification 6.8 to verify ventilation
system performance relative to air balance, as described in the Unit 1 UFSAR for the
following areas: radwaste building, radwaste storage building, turbine building, and steam
tunnel; and (2) failure to establish and implement procedures as required by the Technical

Specification 6.8 to verify ventilation system performance as described in the Unit 2 FSAR
l
relative to periodic testing of the High Efficiency Particulate Air Filter (HEPA) componet of
the Main Exhaust System.
