Reg # 0820 Gentlemen:
We are in receipt of US NRC Annual Haterials Fee Invoice and are questioning what it is for?
We performed a job in the state of Vermont from 8/3/95-9/11/95 and filed the appropriate Form 241 and paid the required $350.00 for QA program review and a fee of S700.00 for reciprocity.
We were not aware of any additional fees required for performing this work.
We were also told at the time, that after that job was finished we could no longer perform any work in an NRC state because the fees we paid were for this job only. We are in question as to what this additional $950.00 is for?
It appears that we may have in fact been able to perform work in NRC states, but we turned it down because we were told by the NRC that the fees we paid were only for this one job.
We are a very small organization, with only 4 full time employees, we can not afford these additional charges and that is why we turned down additional work because we were told we would have to refile and pay the $700.00 fee each time.
What did our check #2347, dated 7/7/95 in the amount of $700.00 cover? We were under the impression this was a materials fee.
We were also instructed to submit Form 241 with the exact date of the job termination. We thought this was to inform the NRC of when we would no longer be performing Radiography in an NRC state. This form was received, signed and dated by someone in the NRC 9/22/95 (see attached).We were under the impression that this informed the NRC that we had completed our assignment.
I would appreciate someone responding to my questions and this new invoice in particular as soon as possible.

Dear lir. Jankovich:
It has recently come to my attention (with the receipt of invoice AH4791-96) that providing date of termination of work in an NRC state on Form 526 is not considered adequate notification to the NRC for termination of QA program.
Effective immediately, please terminate the QA program for Able Testing and Inspection Inc. so there will be no nore surprise invoices for licenses or programs that ue were under the impression were terminated as of September 11, 1995.
It is very confusing uith the different regulations that the NRC has regarding reciprocity notification forms and filing fees, QA Program filing and fees, and then to discover that there is an additional S950.00 yearly fee for each year of the QA program. I can not find anywhere in my paperuork that states the NRC uill invoice QA Program registrants an additional $950.00/ year. Your letter of July 11, 1995 makes no mention of additional yearly QA program fees.
Ifad we knoun that there vould be an additional yearly fee, ue vould have terminated QA Program # 0820 immediately. We uould appreciate any help you could provide us with waiving this S950.00 fee. I look forward to hearing from you regarding this QA Program fee.
