SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-254/96011, 50-265/96011

Dear Mr. Kraft:

On August 22, 1996, the NRC completed an inspection at your Quad Cities
,
i
Nuclear Power Station facilities. The enclosed report presents the results of
that inspection.
.
The inspectors noted indications of conservative decision-making by the
operations staff and management during startup of Unit 2.
Similarly, your
!
decision to complete inspection of numerous concrete expansion anchor bolts
j
prior to starting up both units appeared to be prudent.
~
During this inspection period, you determined that growth of zebra mussels at
the inlet of your-diesel driven fire water pumps had resulted in the pumps
4
being inoperable for the period of May 6 through August 9 of this year. We
,
are concerned that such a situation was allowed to develop and continue to
.
assess the circumstances surrounding this condition. We also noted examples
4
of continued equipment performance problems which appeared to be linked to
poor maintenance practices. We are concerned with this persistent problem,
.
and continue to assess it relative to the corrective actions associated with
'
previous enforcement actions,
i
Examples of human errors committed by maintenance and operations staff
'
continued to be identified during this inspection period. These type of human
,
performance issues have been identified in each of the last five integrated
inspection reports. Three of the human performance problems examined during
this inspection period have been characterized as examples of a violation of
i
your Technical Specifications.
Specifically, a worker failed to adhere to
)
your procedures and began work on the wrong emergency diesel generator cooling
!
water pump, workers failed to adhere to your procedures for rewiring a safety
J
related valve power supply breaker, and workers failed to adhere to your
I
procedures for a safety related pump replacement.
,

The first example was of concern because of the pacntial to cause the loss of
operability of two emergency diesel generators. The second example involved

i
the failure of quality control inspectors to identify the wiring error, while
the third involved a foreman signing for actions which were not performed.

This violation is cited in the enclosed Notice of Violation, and the
I
circumstances surrounding the violation are described in detail in the
/
'
enclosed report.
