NRC INVESTIGATION REPORT NO. 4-96-005 l
Dear Mr. Callan:
My involvement in the December 1995 incident has had a very traumatic effect on both my personal and professional life.
I have tried to rati Ize what led me, and my co-worker to make this error in j udgement.
I have gone over many scenarios of the things that could have contributed to my decision.
Even though many factors may have contributed to my decision, I accept full responsibility for my actions.
The Control Room and Outage Director assured us, at the time we discovered we were on the wrong valve, the valve position was correct, and the integrity of the safety system was maintained.
I erred in not properly documenting the removal and replacement of the motor leads, and not being forthright in reporting this event.
I have lived in the immediate area of Cooper Nuclear Station my entire life.
My family has lived in this area for five generations.
I want to assure the NRC that at no time did I feel the safety of the public, the plant, and my family was compromised when this decision was made.
My co-workers, my friends, my family, and I have suffered tremendously from this incident.
I will never put the industry, a co-worker, or myself in this situation again.
I will strive to be a leader in promoting nuclear safety and integrity.
I request this letter not be made public record.
