SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-373/374/96006(DRP) AND  !
NOTICE OF VIOLATION

,

Dear Mr. Subalusky:

i

From May 25 through June 22, 1996, the NRC performed an integrated team

inspection at your LaSalle facility. The enclosed report presents the results i

of_that inspection.

i

Based on the results of this inspection, the NRC has determined that one >

j violation of NRC requirements occurred. This violation is cited in the '

i enclosed Notice of Violation (Notice) and the circumstances surrounding it are

i

described in detail in the enclosed inspection report. This violation was of . '

concern because. operations department management directed that the containment i

post-accident hydrogen / oxygen (H,/0,) monitors be routinely operated in a i

manner not contemplated in, station procedures and not as described in the  !

UFSAR. In' addition, we are concerned that two previous violations also

pertained to operating in a manner different from described in the UFSAR, and

they-likewise lacked a documented basis for determining that the change did

not involve an unreviewed safety question.

You are required to respond to this letteF and should follow the instructions j

specified in the enclosed Notice when preparing your response. After '

reviewing your response to this Notice, including your proposed corrective

. actions and the results of future inspections, the NRC will determine whether  ;

further NRC enforcement action is necessary to ensure compliance with NRC l

regulatory requirements.
