Modification No. 04 to Task Order No.12 Under Contract No.
NRC-33-92-203 The purpose of this modification is (1) to extend the period of performance from December 31, 1996 through January 31, 1998, (2) to increase the ceiling amount of the task order by $1,226,951.95 from $1,219,815.47 to
$2,446,767.42, (3) to increase the obligated amount by $480,000.00 from
$1,219,000.00 to $1,699,000.00 and (4) to revise the statement of work.
Accordingly, the following changes are hereby made.
The second and third paragraphs of the definitization letter are revised to read as follows:
" Task Order No.12 shall be in effect from August 03, 1992 through January 31, 1998 with a total cost ceiling of $2,446,767.42.
The amount of
$2,352,660.98 represents the reimbursable costs, and the amount of
$94,106.44 represents the fixed fee.
The amount presently obligated with respect to this task order is
$1,699,000.00, of which the amount of $1,633,653.84 represents the reimbursable costs, and the amount of $65,346.16 represents the fixed fee."
