This letter is'in response to Advanced Medical Systems application datd November 27,1995, requesting amendment to the registration certificates for the 3800 Series sources, and the C-9 and V-9 teletherapy units. The information submitted l'n your request did not contain sufficient

information to allow us to process your request; specifically', it is not clear what action AMS is requesting. In our telephone conversation on October 7,1996, you stated that at that time, it was not clear to you what approach AMS wished to take regarding this request, and that you would provide additional information by November 1,1996.
5 To date we have not received any additional information. Please respond within 30 (thirty) days of the date of this letter, informing us of the approach that AMS wishes to take, and providing sufficient information to allow a complete review.
if we do not receive complete information within 30 (thirty) days of this letter, we will be unable to keep your request in an open status, and will consider your application for registration of the previously listed sources and devices as having been abandoned by you. This action would be without prejudice to the resubmission of a complete application. We will keep copies of your application on file for a period of one year from the date of this letter, during which, you may resubmit your application in its entirety without incurring additional fees. Upon receipt of your i
resubmission, we will reenter your request into the system.
