j This is in reference to your request in a letter dated April 5,1984 to renew License Number SNM-1319.
In order to continue our review, we need the following i
additional information:
l 1.
In our cover letter dated July 13, 1983 with Amendment No. 18, we notified you that since your license had been amended 18 times since 1972 that you 4
should plan to submit a complete, up-to-date description of your program.
a Your letter of April 5,1984 does not fulfill our request.
We are now requesting that a complete renewal application be submitted in accordance with 10 CFR 70 without reference to any previous documents.
In submitting this renewal application, please pay particular attention to l
the following:
1.
Please clarify why you have not sent the accountability reports required by Condition 13.B. of your current license for the last 3, 6 month periods, since June 30, 1983.
2.
Please clarify, your current places of use.
)
3.
Please clarify if you currently are implanting pacemakers in dogs.
4.
Please submit a list of persons qualified to handle your licensed material and assemble your pacemakers.
5.
Please clarify the maximum amount of sealed plutonium sources you will have on hand at any one time (milligrams per source, total grams possessed).
l 6.
Please submit a list of instruments used in measuring leak test

performed on your plutonium sources and your leak testing procedures.
7.
Please submit diagrams of your current facilities.
