Nelson Grace SPR 85-008 Page Two July 3, 1985 EVENT DESCRIPTION On June 6, 1985, while performing an Appendix R Audit, three (3) discrepancies were id e n t i f ie d.
1)
A seismic opening in the floor of the Intermediate Bu i ld in g (IB) Room 63-01 was found to be unsealed.

2)
A conduit passing thr ou gh the floor of IB Room 26-02 had a one inch gap between the floor and the triple wrap of knowool.
3)
An error was made in the Fire Watch Log with respect to a room required for the hourly inspections, i.e.,
IB Room 12-07 was d e s ig na t ed in the logs and inspected hourly in s t e ad of IB Room 12-02 as was required.
ACTION TAKEN 1)
Smoke detectors were verified o pe r ab le and a one hou r fire watch patrol was established.
Repair of the opening will be completed dur in g the next outage of sufficient duration, because sca f folding required for ac ces s ibilit y to the opening s.11 impact the operability of the adjacent s a f e t y-r e la t ed equipment.
2)
Smoke detectors were verified o pe r ab l e, and a one hou r fire watch patrol was established.
Repair was completed, and the barrier was declared o pe r ab le on June 10, 1985.
3)
The Fire Watch Log was immediately corrected.
Access to IB Room 12-07 is via IB Room 12-02; therefore, the hourly coverage of the area was in effect accomplished.
