SUBJECT: NRC INSPECTION REPORT 50-445/96-16:50-446/96-16
Dear Mr. Terry:
On December 21,1996, the NRC completed an inspection at your Comanche Peak Steam
Electric Station, Units 1 and 2, reactor f acility. The enclosed report presents the scope
and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities was
generally characterized by safety-conscious operations and sound engineering and
maintenance practices. We are concerned, however, with the operations findings within

Sections 01 and 05 of the enclosed report which reflect operator inattentiveness, potential
supervisory oversight weaknesses, and inadequate self-verification. Since a number of
these issues involve newly qualified reactor operators, we are concerned that the errors
may reflect operator training weaknesses and we plan to monitor the effectiveness of your
corrective actions to prevent these operator performance issues from recurring.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter
and its enclosure will be placed in the NRC Public Document Room (PDR).
Should you have any questions concerning this inspection, we will be pleased to discuss
them with you.
