During reference (a), the Nuclear Regulatory Commission requested that Pearl Harbor Naval Shipyard formally request by letter modification to refer-ence (b) in order to receive and ship an AN/UDM-1A cesium 137 gamma calibrator source / exposure device.

It is requested that the Shipyard's current Quality Assurance Program j
approval be modified and broadened to permit the one-time receipt and shipment of Type B quantities of encapsulated normal form radioactive material in a Specification 55 type shipping container and DOT Specification 20 WC-4 (over-I pack) outer shipping container. This request was previously approved in oar Quality Assurance Program Approval dated 8 February 1984 but was deleted during renewal of our Quality Assurance Program Approval dated 12 September i
