Dear Dr. Bullen:
j-In accordance with my telephone ~ conversation with Scott Wendt; enclosed is the reactor operator license examination, to be administered by you to Mr. Chad Hutchinson on January 13, 1997. At the conclusion'of the examination, please return the entire examination and t.nswer sheets in the enclosed envelope.
Please ensure that the examination is proctored (the time limit is one hour) and that the candidate signs the statement at the bottom of the cover sheet at the conclusion of the examination.
I have enclosed a copy of our Guidelines for. Administering Written Examinations for your guidance.
Note that this is a closed-reference examination, and there should be no notes, charts or other training materials open in the examination room.
Calculators and scratch 1
paper are allowed.
If any questions regarding the examination test items arise, please respond by telling the candidate to answer the test item as best he can. Make note of the question so that we may discuss it after the examination.
If there are any major problems, I can be reached at (301) 415-1833.
This is the second time that a retake examination has been administered at your facility in this manner. We hope that we may continue this process in order to realize cost savings and efficiencies.
