SUBJECT:
NOTICE OF VIOLATION AND PROPOSED IMPOSITION OF CIVIL PENALTY: EA 85-45 Within the time and manner specified, St. Thomas Hospital failed to answer the Notice of Violation and Proposed Imposition of Civil Penalty sent to you with our letter dated May 14, 1985. Your response was due June 13, 1985.
Discussions with Dr. Kab Kim on July 10, 1985, indnated that the hospital was unable to locate the documents and, perhaps, had not received them.
- However, this office verified with the courier company which delivered the documents that they had been received by your office at 4:45 p.m. on May 16, 1985.
You are required to provide a complete response to the Notice of Violation and Proposed Imposition of Civil Penalty (enclosed) within thirty working days of the date of this letter. This additional time and attached courtesy copy are provided to you to ensure that you have a copy of the document and time to provide a response.
Failure to provide a response may result in adattional enforcement action.
