As a result of the inspection conducted on August 29, 1985, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violations were identified:
A.
10 CFR 31.5(c)(2) requires that the device be tested for leakage of radioactive material at no longer than three year intervals.

Contrary to the above, as of August 29, 1985, sealed sources containing 200 millicuries of cesium-137 had not been tested for leakage since August 1980, an interval of more than three years.
This is a Severity Level IV violation (Supplement VI).
B.
10 CFR 31.5(c)(1) requires that all labels affixed to the device at the time of receipt and bearing a statement that removal of the label is prohibited are maintained thereon and shall comply with all instructions

and precautions provided by such labels.

Pursuant to the provisions of 10 CFR 2.201, Formosa Plastics Corporation is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achievec; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
