- 1. Purpose The purpose of this program is to provide paramete-s and methods to be used in order to comply with the requirements of"10 CFR Part 71"
- 2. San The procedures contained herein shall be used by all MIDWEST INSPECTION SERVICES personnel invoh>cd in use and transportation of radioactive material shipping packages.

OWNERS - RAYMOND EZZELL AND DAVID E72 ELL RADIATION SAFETY OFFICERS - LINDY " SKIP" GLENN, RAYMOND F77F1 L DAVID F77RLL A. The final responsibility for the Quality Assurance Program for Part 71 requirements rests with Midwest Inspection Senices.
B. The Radiation Safety Oflicer (RSO) is the responsible individual for assuring compliance with part 71 Q.A. requirements. In addition, the RSO is responsible for overall administration of the j
program, training and certification, document control, and auditing.
C. The radiographers are responsible for the safe handling, storing, shipping, inspection, test and operating status and the applicable record keeping.
- 4. Ouality Assurance Program A. The management of MIDWEST INSPECTION SERVICES is responsible for establishing and

implementing this Q.A. program. Training, prior to engagement, for all Q. A. functions required in accordance with MIDWEST INSPFCTION SERVICES Personnel Certification Procedure SP-PC-4. Q. A. revisions will be m ade a xording to written procedures with management approval.
The Q.A. program will ensure that 114 fined Q.C. procedures, engineering procedures and specific l
provisions of the package design approval are satisfied. The Q.A. program will emphasize control of the characteristics of the package which are critical to safety, B. The Radiation Safety Officer shall ensure that all radioactive material shipping packages are a
designed and manufactured under a Q.A. program approved by the Nuclear Regulatory Commission for all packages designed or fabricated after January 1,1979. This requirement will be satisfied by receiving a certification to this effect from the manufacturer.
- 5. Document Control A. All documents related to a specific package will be controlled through the use of written procedures. All document changes will be performed according to written procedures approved by management.
B. The Radiation Safety Officer shall ensure that all Q.A. functions are conducted in accordance with the latest applicable changes to these documents.

- 6. Handline. Storare, and Shinoine l
A. The written safety procedures concer:nng the handling, storage, and shipping of packages for certain special form radioactive material will be followed. Shipments will not be made unless all tests, certifications, acceptances and final inspections have been completed. Work instructions are provided in MIDWEST INSPECTION SERVICES Operating and Emergency Procedures.
B. Radiographic personnel shall perform the critical handling, storage and shipping operations.
- 7. Inspection. Test And Operatine Status A. The inspection, test and operating status of packages for certain special form radioactive materials will be indicated and con. rolled by written procedures. The status will be indicated by the tag, label, marking, or log entry. The status of non-conforming parts or packages will be positively maintained by written procedures.
B. Radiographic personnel shall perform the regulatory required inspections and tests in accordance with written procedures. The Radiation Safety Officer shall ensure that these functions are performed.
- 8. Ouality Assurance Records A. Records of package approvals, procurement, inspections, tests, operating logs, audit results, personnel training and qualifications, and records of slupments will be maintained. Descriptions of equipment and written procedures will also be maintained.
2 B. These records will be maintained in accordance with written procedures. The records will be identified and retrievable. A list of these records, with their storage locations, will be maintained by the Radiation Safety Officer.
- 9. Auditt Established schedules of audits of this Q. A. program will be performed using Quarterly Field Audit Checklist (page 3). Results of audits will be maintained and reported to management. Audit reports will be evaluated and deficient areas corrected. The audits will be dependent on the safety significance of the actisity being audited, but each activity will be audited at least once per year. Audit reports will be maintained as part of the quality assurance records. Members of the audit team shall have no responsibility in the actisity being audited.
