NRC INSPECTION REPORT NO. 50-295/96013(DRS); 50-304/96013(DRS)
Dear Mr. Mueller:
This refers to the inspection conducted by M. Holmberg and K. Green-Bates of this office
on October 1-3,28-30, and November 7,1996, of activities authorized for your Zion
Generating Station, Units 1 and 2. The focas of this inspection was your inservice
inspection efforts. At the conclusion of the inspection, the findings were discussed with
those members of your staff identified in the enclosed report.
Based on the results of this inspection, certain of your activities appeared to be in violation
<
of NRC requirements, as specified in the enclosed Notice of Violation (Notice). The first
violation is of concern because it represents nonconservative decisions to return potentially
defective steam generator tubes to service following refueling outages in 1995. The
second violation is of concern because it is indicative of staff weaknesses in knowledge
and implementation of the ASME inservice inspection program.
During ine recent 1996 Zion Unit 2 refueling outage, your staff performed extensive steam
generator tube eddy current testing using conservative data analysis guidelines. Your staff
also performed extensive in-situ tube leakage / burst tests and five tubes will be removed for
metallurgical analysis. Based on this performance, we concluded there has been an
improved safety focus in managing steam generator tube degradation when compared to
the 1995 refueling outages.

You are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. In your response, you should document
the specific actions taken and any additional actions you plan to prevent recurrence. After
reviewing your response to this Notice, including your proposed corrective actions and the
results of future inspections, the NRC will determine whether further NRC enforcement
action is necessary to ensure compliance with NRC regulatory requirements.
