Dear Mr. Fogle:

During a recent inspection conducted by members of my staff at the Frederick Cancer Research Center, we noted that your company had performed work on the source drive mechanism of a J. L. Shepherd Model 1 irradiator.

We have verified with the State of Maryland, Division of Radiation Control that your current Maryland State License only authorizes you to perform work on teletherapy units. Therefore, you must amend your Maryland State License to authorize service on specific irradiators in order to perform this work at Federal facilities and in states in which the Nuclear Regulatory Commission maintains jurisdiction of the use of byproduct material.

In addition, you must file a properly filled out Form NRC-241 prior to conducting the work.
