SUBJECT: NRC !NSPECTION REPORT 50-382/96 13 AND NOTICE OF VIOLATION

Dear Mr. Sellman:

An NRC inspection was conducted October 13 through November 30,1996, at your

Waterford Steam Electric Station, Unit 3 reactor facility. The enclosed report presents the

scope and results of that inspection.

During the 7-week period covered by this inspection period, your conduct of activities at - ]

Waterford was generally characterized by safety conscious performance of maintenance,

engineering, and plant support, in the area of operations performance, concerns were

identified with recognition and implementation of Technical Specificatien limiting conditions

for operation, configuration control, and inadequate control room panel walkdowns. l

There were three violations of NRC requirements that were identified. The first violation is

of concern because procedures were not followed regarding configuration control. This

violation represents multiple examples of system configuration discrepancies which were i

not identified by your operators and existed through a .mmber of shift changes. These

situations highlight the need for significant improvement in the attentiveness of your

operators. The second violation is of concern because operators failed to recogn:'ze that

wet cooling tower fans were rendered inoperable during a maintenance activity and,

therefore, the appropriate Technical Specification limiting condition for operation was not

entered. The failure to enter the appropriate Technical Specification limiting conditions for

operation has been a recurring problem at your facility. The third violation is of concern l

because your review of the inservice testing program failed to identify the need to perform I

testing on dry cooling tower isolation valves, which are required to be repositioned during

the design basis tornado event.

These vio:ations are cited in the enclosed Notice of Violation (Notice) and the

circumstances surrounding the violations are described in detailin the enclosed report.

Please note that you are required to respond to this letter and should follow the

instructions specified in the enclosed Notice when preparing your response. The NRC will

use your response, in part, to determine whether further enforcement action is necessary

to ensure compliance with regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, l

its enclosure (s), and your response will be placed in the NRC Public Document j
