APPENDIXA Radioactive. Effluent Monitoring Instrumentation Requirement: Radioactive effluent monitoring instrumentation channels are required to be operable in accordance with ODCM Sections 2.3.3 and 2.3.4. With less than the minimum number of channels operable and reasonable efforts to return the
, instrument (s) to operable status within 30 days being unsuccessful, ODCM Sections 2.3.3 and 2.3.4 requires an explanation for the delay in correcting the inoperability in the next Annual Effluent Release Report.
Response
The Service Water monitor was declared inoperative on February 28,1995 and 4
remained out-of-service for the remainder of the year. The plant was in an extended shutdown throughout this period. For much of that time, there was an absence of vacuum priming which is required to supply sample to the monitor. Once vacuum priming was restored, it was discovered that the sample pump was inoperable. That pump was subsequently repaired.
The Primary Vent Stack Flow rate meter was declared inoperative on July 5,1995.
Delays were encountered in the repair of this stack mounted instrumentation due to the unsafe conditions identified with the access ladder / platform. Once those conditions were repaired, the instmmentation was inspected and found to be obsolete and the necessary repair parts were no longer available. A replacement unit has been ordered, but delayed due to a long purchase lead time.
The Primary Vent Stack noble gas monitor (RM-3903Y) is required to be calibrated
- each refueling interval. By administrative error, the monitor was inadvertently not calibrated within the required interval including the +25% time allowance, which expired in July 1994. Subsequently, the monitor was successfully calibrated and found to be within tolerance in March 1995. During the period July 1994 to March ;

1995, the monitor, therefore, is considered to have been inoperative with regard to the above requirement. Throughout that period, daily grab samples of the Primary ,
Vent Stack gas were collected and analyzed. The required remedial action defined '
in the ODCM for an inoperative monitor was, therefore, satisfied.
