Approval No. 0851, expired on November 30 1996. i If you are an NRC licensee and conduct activities under the General Licenses of Subpart C of 10 CFR Part 71. or an Agreement State Licensee subject to the requirements of 10 CFR Part 71. as required by 10 CFR S 150.20 a OA Program approved by the Commission as satisfying the provisions of Subpart H of 10 CFR i Part 71 is required. Since you no longer have a valid QA Program Approval. ;
the General Licenses of Subpart C of 10 CFR Part 71 and registration under the provisions of 10 CFR S 71.12. to transport or deliver licensed material to a carrier for transport are terminated.
Please note that records required by 10 CFR SS 71.91 and 71.135 are to be retained for a period of three years. .
