Advanced Medical Systems Inc. wishes to renew Certificate of Compliance No. 9049.
Enclosed you will find ten copies of a consolidated application in our name.
This application is largely based upon information provided in the previous application made by General Electric Co., dated February 21, 1980.
There have been no changes in design of the package since that time.
Advanced Medical Systems purchased the package from GE in 1983 and has been using it since that time.
I have enclosed a check in the amount of $150.00 to cover the application fee per 10CFR170.21(10).
It is my understanding that this application will fall under the timely renewal provisions of 10CFR2.109.
