Please review the enclosed document carefully and be sure that you understand

all conditions. If there are any errors or questions, please notify us so

that we can provide appropriate corrections and answers.

Although your procedures now specify that laboratory areas muct be surveyed

by your personnel after each use of licensed material, your Radiation Safety

Officer (RS0) should also conduct independent laboratory surveys at a

specified frequency. The surveys should include both radiation level and

contamination wipe tests. You should submit the procedure that your RSO uses

within the next 30 days.

Any future correspondence relating to your license should specifically

reference your license and docket numbers to expedite your inquiry.
