NRC OFFICE OF INVESTIGATIONS REPORT NO. 3-2019-003

Dear Mr. Dagres:

This letter refers to an investigation conducted by the U.S. Nuclear Regulatory Commission (NRC), Office of Investigations (OI) to determine (1) whether a quality assurance (QA) specialist, HydroAire Services, Incorporated (HydroAire) in Chicago, Illinois, and an Authorized Nuclear Inspector (ANI), OneCIS Insurance Company (OneCIS), deliberately signed a false hydrostatic test report; (2) whether an ANI Supervisor, OneCIS, directed a QA manager, HydroAire, not to write a corrective action report; and (3) whether an ANI, OneCIS, deliberately certified hydrostatic tests that were not properly completed. Based on the evidence developed during the OI investigation, the NRC did not substantiate these concerns.
