Please address the following:
1.
Describe all design changes between the specifically licensed (SL) and the generally licensed,(GL) devices.
2.
Provide a description or drawing of the GL label and indicate its location on the device.
Please verify that the E
GL label will not be present on the SL devices.

Verify tnat the label will have the date of assay for the GL devices.
4.
List any services, maintenance or testing that you will provide.
5.
List any servicing, maintenance or testing that you want the GL user to perform.
6.
Please provide a copy of the users manual.
We do not have a copy in the file, and we need to review the manual for GL users.
7.
Provide changes'and/or additions to the QA/QC procedures i
that'will be performed for the GL device that will assure
'that all GL devices contain the appropriate design features i
