This will serve as a request to renew our IIcense number SNM-1551, docket number 070-01993 Our present mailing address is: The Lutheran Hospital of Fort Wayne, Inc., 3024 S. Fairfield Avenue, Fort Wayne, Indiana 46807 Presently, we have four (4) living patients with Plutonium pace-makers implanted, two (2) patients have Medtronics model 9000, one (1) patient with Coratomics Model 100, and one (1) patient with Coratomics model 101. No Plutonium pacemakers remain in our possession.
The principal reason for our request to renew the existing license is to' follow patients with implanted devices and to return explanted pace-makers to the manufacturer for proper disposal (but not to implant Plutonium pacemakers in the future).
Our listing of physicians remains as indicated in condition 16 of our license, August Tomusk, M.D.: Michael Mastrangelo, M.D.,
and Wayne Miller, M.D. In practice at 2828 Fairfleid Avenue, Fort Wayne, Indiana 46807 Please see copy of IIcensed attached.
