As required by 10CFR71.12 the University of Virginia Reactor Facility hereby requests that it be added to the list of authorized users of the ETR Fuel Element Shipping Containers, Package ID Number USA /9134/B( )F. Our license numbers are as follows:

License No. R-66 Docket No. 50-62 CAVALIER License No. R-123 Docket No. 50-396 Our facility meets all the requirements of 10CFR71.12 in that we have an approved quality assurance program, have a copy of the license, certificate of compliance and other documents relating to the use of the package.
Please forward to us approval to use the cask as soon as possible as we intend to use it to ship unused fuel from our facility as will be mandated by a recent decision of the Commission.
