SUBJECT:

EXPIRED LICENSE, ST. AGNES MEDICAL CENTER, PHILADELPHIA, PA On January 8,1985, Dr. Frederick DeClement, the licensee's Clinical Professor of Surgery, was contacted and informed that License No. SNM-1767 had expired on August 31, 1982 and that no renewal application had been received. The license representative explained that they had previously possessed licensed material but no longer possess any licensed material and wish to have the license terminated.

He stated that the licensee had returned the nuclear pacemaker to the manu-facturer, Medtronic, Incorporated. This was documented in a properly completed NRC Form 314 from the licensee dated February 27, 1985. Marlene Taylor, of my staff, discussed this matter with Ms. Mary Obert, Return Products Coordinator of Medtronic, Incorporated, License No. SNM-1156, who confirmed that they had received the nuclear pacemaker from the licensee on April 13, 1979, and that they are authorized to possess this material.

Based on a review of the information supplied by the licensee and a review of the type and form of material authorized by the license, we have determined that a closecut survey by the licensee is not required and have retired License No. SNM-1767. Please remove it from your active shelves.
