SPEC Consultants, Inc. hereby requests renewal of its Q.A. Program approval for radioactive material packages No. 0612 which expires on October 31,1996 This renewal is requested for use with SPEC Consultants, Inc. Radioactive materials license 37-27891-01 for Industrial Radiography which does require shipment in accordance with 10 CRF Part 71.
Since there is no fee required, no remittance is included.
