SUBJECT: NRC INSPECTION REPORT 50-295/304/96010(DRP) l

Dear Mr. Mueller:

On August 29, 1996, the NRC completed an inspection at your Zion 1 and 2

react: 'acilities. The enclosed report presents the results of that

'

During this six-week inspection period, the Zion facilities experienced the

second occurrence this year of overflowing the OB lake discharge tank, cross-

tying the auxiliary steam and service air systems, and several other weak

operations, maintenance, and engineering practices described in the report.

Radiological work controls were adequate.

In addition, four violations of NRC requirements were identified. An

inadequate fuel handling procedure was used during new fuel assembly receipt  !

inspection, which resulted in dropping two new fuel assemblies two inches. 1

The inspectors found a scaffold that interfered with operation of safety

related equipment. In addition, a missing part from the IB centrifugal

charging pump was found during a pump inspection but the discrepancy was not l

properly documented.

These violations are cited in the enclosed Notice of Violation, and the

circumstances surrounding the violations are described in detail in the

enclosed report. Please note that you are required to respond to this letter

'

and should follow the instructions specified in the enclosed Notice when

preparing your response. The NRC will use your response, in part, to

'

determine whether further enforcement action is necessary to ensure compliance

alth regulatory requirements.

One additional violation concerning the second occurrence of overflowing the

OB lake discharge tank does not require further response. Your response to

the August 23, 1996, Hotice of Violation adequately addressed this matter.
