SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-341/96006  ;

Dear Mr. Gipson: l

On August 2, 1996, the NRC completed an inspection at your Fermi 2' reactor l

facility. The enclosed report presents the results of that inspection, j

Areas examined during the inspection are identified in the report.- Within~  !

these areas, the inspections consisted of a selective examination of  ;

procedures and representative records, interviews with personnel, and i

observatics of activities in progress. The purpose of the inspection effort j

was to determine whether activities authorized by the license were conducted

safely and in accordance with NRC requirements. l

!

This inspection report represents an integrated product of a number of diverse j

inspections that were conducted during the inspection period. The summary  ;

contained in the report provides an overview of our assessment of your  !

performance in the four functional areas (Operations, Maintenance, i

Engineering, and Plant Support) in which inspections were performed.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of-

Violation. Specifically three violations of NRC requirements were identified.

In the first instance, corrective actions for a previously cited violation '

were insufficient to prevent recurrence in that your staffs monitoring of ,

safety related service water system performance failed to identify repeat  :

fouling of a drain line. In the second violation, workers . failed to follow

site procedures by improperly staging equipment in the Division II Battery .

The third violation involving the failure to include a valve in the Residual  !

Heat Removal Service Water System Operating Procedure was of' concern because l

it was a another example of inadequate procedures. Problems with inadequate  ;

and incorrect procedures were documented in Inspection Reports 96002, 96004,

l and 96005. _ In your response to the third violation, you are requested to ,

l

discuss corrective actions implemented in response to the continuing trend of j

inadequate procedures and provide a status of these actions. t

circumstances surrounding the violations are described in detail in the

l enclosed report. Please note that you are required to respond to this letter

and should follow the instructions specified in the enclosed Notice when

preparing your response. The NRC will use your response, in part, to deter-

mine whether further enforcement action is necessary to ensure compliance with

l regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

l this letter and its enclosures will be placed in the NRC Public Document Room

l (PDR).
