This is a request under the Feedom of Information Act as amended (Title 5, U.S. Code, Section 552).
I wish to obtain copies of all documents in your files pertaining to licensing, inspection and investigation of Mid-States Logging and Perforating Co. and United Wire Line Co., both of which are located in Fairfield, Ill.
Mid-States license number is 12-199-8501 and United Wire Line is 12-423-2001.
I am particularly interested in an affidavit written by Don Ghirardini, manager of Mid-States I am also on May 18.regarding a missing radioactive source.
interested in files reagarding licensing and insoection processes.
I am also interested in records regarding Carl Hubbart, manager and of United Wire Line and Don Gherardini, manager of Mid-States, their histories prior to their employment with their current companies.
I am most interested in records dated January 1984 and later.
If all or any part of my request is denied, please list the soecific exemption on which you are relying to withhold the information.
For any " classified" material withheld, please include the classification, identity of the classifier, date or event for automatic declassification, classification review, or downgrading and, if applicable, the identity of the official authorizing extension of automatic declassification or review past six years and the reasons for l
extended classification.
If you determine that portions of the requested material are exempt from release, I will expect, as the FOIA requires, that you provide me with the remaining, non-exempt portions.
