It has come to our attention that the address for Longview Inspection Gulf Coast Group, Inc., as shown on Quality Assurance (QA) Program Approval for Radioactive Material Packages No. 0771, Revision No. O, may have changed.
If the location where you keep your QA records or conduct transportation activities has changed, you thculd request an amendment to your QA Program indicating the change of address and any other changes that may have been made since your QA Program was approved. A fee of $250 should be included with your request for this amendment.
