Subject:
Modification No. 1 to Task Order No. 6 Under Contract No. NRC-26-94-263 This confirms the July 1, 1996 and August 20, 1996, verbal authorizations provided to you to allow for additional' students in the courses as stated under items no. 1 and 2 below and provides funds accordingly.
This modification also ratifies the actions taken to allow for additional students in the courses stated under items no. 3 and 4 and covers additional travel costs incurred by you in the performance of this work and provides funds accordingly.
These changes are made as agreed to per Conger & Elsea's September 26, 1996, letter which is attached hereto and made a part hereof.
I The purpose of this modification is as follows:
1.
Revise the established course date as stated in the SOW for this task order, for the Region I March 11 - 15, 1996, " Root j
Cause / Incident Investigation Workshop" to July 15 - 19, 1996 and increase the number of students by 1 from 15 to 16.
This is an increase of $1,920.00 in the cost of the course presented.
I 2.
Revise the established course date as stated in the SOW for this task order, for the Region I March 10, 1996, " Human Performance Investigation Process Workshop" to September 25, 1996 and increase the number of students by 6 from 15 to 21.
l This is an increase of $720.00 in the cost of the course I
presented.
3.
Increase the number of students for the Headquartars May 20-24, 1996, " Root Cause/ Incident Investigation Workshop" by 3

This is an increase of $5,760.00 in the cost of the course presented.

Increase the ceiling for travel costs by $9,756.00 from
$11,977.00 to $21,733.00 to allow for non-per diem rates costs and higher airfare rates costs incurred by the contractor in the course of performing this effort.
The above total additional costs incurred by the Contractor of
$29,467.00 are hereby provided as incremental funds which thereby increases the ceiling and obligated amounts of this task order from $122,245.00 to $151,712.00.
Accordingly, paragraph no. 3 of Task Order No. 6 dated March 15, 1996, is hereby deleted in its entirety and the following substituted in lieu thereof:
" Task Order Nn 6 shall be in effect from March 10, 1996, through Septeinber 30, 1996, with a ceiling of $151,712.00 which is inc'.usive of the $2,102.00 authorized on March 4, 1996.
The amount of $129,979.00 represents the total fir.n-fixed-price for the courses as stated in the enclosed Statement of Work.
The amount of $21,733.00 represents the total estimated reimbursable costs for travel associated with this task order."
The obligated amount of this modification is $29,467.00.
