This is in reference to your request in a letter dated November 30, 1984 to renew License No. SNM-1504.
In order to continue our review, we need the following additional information:
Item 1: Your letter requests authorization for Coratomic and Medtronics pacemakers.
In addition to these manufacturer's, you are also currently licensed for the ARCO Nuclear Model NU-5 pacemaker (items 6A, 7A, 8A, and 9A of your license).
In order to delete this item from your-license the following conditions must be satisfied:
(A) All ARCO Plutonium pacemakers that were received have been returned to the manufacturer before implant in a patient, and/or (B) All ARCO Plutonium pacemakers implanted in patients have been recovered and returned to the manufacturer.
Please specify if the above conditions have been satisfied.
We will continue our review upon receipt of this information.
Please reply irl duplicate to my attention at the Region I office and refer to Mail Control No. 03220.
