INCLOSIIRE Tmamediv, Incorporated License No. 29-15354-02 Certain activities under your license appear to be in violation of AEC requirements.
A.
The violations considered to be of Cat.egory II severity are as follows:

10 CF3.30.3, " Activities requiring license," requires that except for persons esempt as provided in this part and Part 150 of this -
chapter, no person shall manufacture, produce, transfer, receive, acquire, own, possess, use, import or export byproduct material except as authorized in a specific or general license issued pursuant to the regulations in this chapter.
License Item 7.A through 7.D authorizes AECL Model C-188 cobalt 60 sealed sources and various small cobalt 60 sealed sources for instrument calibration.
i, Contrary to the above, your inventory of byproduct material included approximately 8,000 curies of cobalt 60 as sealed sources which were
- obtained from Gamma Process Company. These sealed sources are not authorised.by this license.

License Condition No. 13.A states each sealed source cont =4nine
'I byproduct material shall be tested.for leakage 'and/or contamination at intervals not to exceed six months. In the absence of a certifi-
. esta from a transferor indicating that a test has been made within six manths prior to the transfer, the sealed source shall not be i
put into use until tested.
Contrary to the above, the AECL cobalt 60 sealed scurces were put into use without being leak tested and without a certificate from tfie transferor indicating that a test had been made within six months prior to the transfer.

License condition No. 14 states, in part, the procedures contained in AECL's instruction==numi for the "IR68 Cobalt 60 Irradiation thiit" shall be followed. The instruction manual contains emergency instructions which, according to the manual, should be posted in a pra=4==nt position close to the control station.
Contrary to the above, the smergency instructions were not posted at the time of this inspection.

" Posting of notices to workers," requires in part, that each licenses post current eqpies of 10 CFR Parts 19 and 20, the licamme, and related doen===ts and procedures referenced in the liosase. If posting of the above documents is not practicable, a notion may be posted which describes the documents and statss share they may be====4==f.
Contrary to the shove, the documents identified above were not posted nor was say notice posted which described where they.nay be found.
